Immunization Requirements for Entrance:
A child entering the school district of Walton County for the first time must present 1 of the 4 certificates below:
-DOH Form 680 (part A-1 or A-2) Florida Certificate of Immunization for diphtheria/pertussis/tetanus (DPT), mumps/measles/rubella (MMR), poliomyelitis, hepatitis B series, varicella (two doses for Kindergarten entry beginning 2008-2009 school year and every subsequent year the next grade up) Seventh grade must have a Td booster.
-DOH Form 680 (part B) Florida Certificate of Immunization Temporary Medical Exemption or a written exemption (not to exceed thirty school days) issued by an authorized official to permit a child who transfers into the district to attend classes until his records can be obtained. At the end of the thirty day exemption period, if the parent or the student fails to present a proper immunization certificate, the principal is to exclude the student until proper immunization documentation is presented to the school.
